BLUE CROSS / BLUE SHIELD UPDATE

Some of you have been advised and have even had claims erroneously denied
under the misconception that we do not "Well Care" for all members of the family.
To that end many of our ID cards are marked improperly. The fact is we do and as
an FYI Well Care includes physicals, immunizations and lab work.

The foregoing has been corrected to our satisfaction and the process that has your
claims being paid properly has begun. If you have had a problem please do any of
the following: Call 1-800-355-2583 asking the BC/BC representative to re-process
your claim or, have the service provider resubmit for payment. As a last resort you
can copy the Explanation of Benefits form and then forward same to the attention
of Dawn Hoffman at City Hall.
